Insurance:

We accept most major insurance plans. If you are unsure whether we are an in-network provider for your specific plan, please call your insurance carrier prior to your appointment to avoid incurring unintended charges not covered by your insurance. If a referral is required by your insurance carrier, please request a referral from your primary physician several days before your appointment to ensure plenty of time of processing. Copayments vary based on your insurance plan, and we collect a specialist copayment. Copayments are due at the time of service.

Lateness/No Shows:

Because of the high volume of patients in our office, anyone over 15 minutes late will have to be rescheduled for a later date. If you are unable to make your appointment or if you are going to be late, please call to make arrangements. Same day cancellations will be assessed a missed appointment fee of $25.

Planning Your Visit

Please plan to arrive 30 minutes before your scheduled appointment time

 Please Be Sure to Bring:

  • Insurance Card
  • Drivers License or Photo Identification
  • Current Medications
  • Forms

Forms:

If you have questions regarding these forms, they can be addressed when you check in for your appointment.

> New Patient Packet

> New Patient History

> Release of Medical Records

Medications:

We ask that you bring a complete list of medications and drug allergies to every appointment. Most patients have more than one physician prescribing medications, and it is very important that we keep an updated list in your chart to avoid unnecessary side effects and drug interactions. If you are ever prescribed a new medication and are unsure if it is safe to take with a medication we prescribed, please check with the pharmacist or call our office.
